Short Construct Revision
Patient History:
- 53 y/o female; BMI: 33.5; T-Value: -0.1
Presented with:
- Low back pain
- Bilateral L5/S1 radiculopathy
- Severe L5/S1 spondylosis and grade 1 anterolisthesis
Initial Treatment & Outcomes:
- L5/S1 TLIF & pedicle screw fixation
- Axial back pain radiating into bilateral buttock and groin
- No SI joint pain, no obvious radiculopathy
- Radiographic evidence of L5-S1 loosening & pseudarthrosis
Surgical Decision Making:
- Additional fixation/stability needed – extension of construct to L4 and pelvis
- Improve pedicle screw purchase:
- Fenestrated pedicle screws & cement augmentation at L4/L5
- Bicortical purchase of implants at S1 level in lieu of fenestrated pedicle screws and cement augmentation
Surgical Treatment:
- Removal Instrumentation L5 – S1
- Fusion/revision fusion L4 to sacrum, instrumentation L4 to pelvis
- Bilateral placement of iFuse Bedrock Granite®:
- S1 Pedicles: 9.5 x 40 mm
- S2AI: 9.5 x 80 mm
Post-op (3-month follow-up):
- Pain was constant 10/10 on the VAS, and improved to intermittent 3/10 at follow-up1
