Short Construct Revision

Patient History:
  • 53 y/o female; BMI: 33.5; T-Value: -0.1

Presented with:
  • Low back pain
  • Bilateral L5/S1 radiculopathy
  • Severe L5/S1 spondylosis and grade 1 anterolisthesis

Initial Treatment & Outcomes:
  • L5/S1 TLIF & pedicle screw fixation
  • Axial back pain radiating into bilateral buttock and groin
  • No SI joint pain, no obvious radiculopathy
  • Radiographic evidence of L5-S1 loosening & pseudarthrosis

Surgical Decision Making:
  • Additional fixation/stability needed – extension of construct to L4 and pelvis
  • Improve pedicle screw purchase:
    • Fenestrated pedicle screws & cement augmentation at L4/L5
    • Bicortical purchase of implants at S1 level in lieu of fenestrated pedicle screws and cement augmentation

Surgical Treatment:
  • Removal Instrumentation L5 – S1
  • Fusion/revision fusion L4 to sacrum, instrumentation L4 to pelvis
  • Bilateral placement of iFuse Bedrock Granite®:
    • S1 Pedicles: 9.5 x 40 mm
    • S2AI: 9.5 x 80 mm

Post-op (3-month follow-up):
  • Pain was constant 10/10 on the VAS, and improved to intermittent 3/10 at follow-up1
