  1. Co-Marketing Agreement (if not already on file with Medical Affairs)
  2. Expense Tracking Form (estimated fees)
  3. Surgeon’s confirmation of fee splitting amount(s)
  4. Co-Marketing Invitation
  5. Newspaper Ad
  6. Post Card Mailer
  7. Promotional Flyer
  8. Slide Deck Presentation
  9. Any other relevant document or script(e.g., radio ad)

After the scheduled Co-Marketing Program

  1. Send final Expense Tracking Form with itemized receipts to Medical Affairs. The final Expense Tracking Form must be acknowledged as accurate by the Speaking Physician.
  2. Submit event sign in sheet to Medical Affairs. The sign in sheet must capture the signature of every attendee at the event.
  3. Upload the Expense tracking Form, itemized receipts and a copy of the sign in sheet to Concur
