SI-BONE Grant Requests

SI-BONE, Inc. may provide financial support to third-party educational events and conferences that further medical and scientific knowledge. SI-BONE believes that such programs are critical to advancements in the medical community, and we support a wide range of programs at the local, regional and national levels.

SI-BONE provides grants to support the following types of educational programs:

  • National, regional or local conferences and professional meetings, eligible for continuing medical education (CME) credit or otherwise
  • Grand Rounds presentations
  • Publication or rebroadcasts of a CME Conference or program (in booklet or pamphlet form, over the Internet, etc.).
  • Other educational materials or initiatives

Grant Application Process Overview

  1. Requestor completes the form below
  2. The SI-BONE Grant Committee will review all grant requests submitted and make a decision to approve or deny the request to ensure compliance with SI-BONE’s Compliance Policy and the AdvaMed Code of Ethics. The review process may take up to 60 days. If your request is approved, SI-BONE will provide notification to you and will send a check to the address you provided on your form.

  1. Conference title, date(s), and location

  2. Substantive topics to be covered including times, speakers, and conference faculty names

  3. Description of expected attendees

  4. If accredited, the accrediting entity's name, with the number of education credits applied for/approved for the conference

  5. Budget detail for the conference or program that supports the amount requested below

Funding and Other Support Information

  1. Amount requested from SI-BONE

  2. Other BSC support requested (including free product, supplies, etc.)

  3. If this is a recurring event, SI-BONE monetary and non-monetary support provided in prior years, if any

  4. Number of attendees last year
    Number of expected attendees this year

  5. Is exhibit space included with sponsorship? Yes No

  6. Is applicant owned or operated by the U.S. Federal Government or a State or Local Government? Yes No

    If so, please provide the name of the agency/institution and please indicate whether all appropriate ethics officer approvals have been obtained to receive this grant.
